Guten Tag! In this ECG recording, the QRS voltage is changing from one beat to another. This is called Electrical Alternans of QRS complex and is seen in patients with Pericardial Effusion/Tamponade.

AC is alternating current; this is electrical alternans, suggestive of pericardial effusion. Electrical alternans is alternating amplitude of QRS complexes in any or all leads.
