1 in 2200

Prenatal ultrasound showed a "heterogenous mass" in the chest concerning for "cystic adenomatoid malformation." After delivery, this X-ray was obtained due to a barrel chest and scaphoid abdomen.

Challenge: What do you think of the prenatal ultrasound interpretation?

    This is a congenital diaphragmatic hernia. The X-ray shows air-filled bowel loops in the left chest and mediastinal shift to the right. The neonate likely also has pulmonary hypoplasia. On prenatal ultrasound, bowel contents can be differentiated from cystic adenomatoid malformation if peristalsis is seen.
