Whoops, I need to go change my operative dictation so that it no longer says, "Sponge and instrument counts were correct x2." My bad!!!!

Risk Management
These are retained foreign bodies; the AP x-ray of the pelvis shows a retained needle and the CT scan shows a retained surgical sponge in the liver.
