The hyperacute MCA sign refers to the appearance of increased attenuation of the proximal portion of the MCA. It reflects thrombosis of the M1 MCA segment and is a sign of ischemic stroke. Other early signs of ischemic stroke seen on non-contrast head CT include hypoattenuation of the brain tissue, loss of the gray-white junction, loss of sulci, and the insular ribbon sign.
