Well?

ECG Wave Maven has been my go-to resource for great EKG's. Created by Beth Israel Deaconness Medical Center in 2001, it has an incredible number of fascinating cardiograms, cases, and explanations. It's an unsurpassed repository for learning and education, and I hope you check it out.

This EKG comes from a middle aged man with vague chest pain. Initial troponins are negative.

Thought to be due to reperfusion of a previously clotted LAD

NSR with single (unifocal) PVC. ??Some nonspecific ST segment changes (T wave inversion in the precordial leads)

This is Wellens’ syndrome. The worrisome finding is biphasic ST-T waves in the precordial leads V1-V4. These segments demonstrate elevations that are arched or bowed upward followed by prominent negative T waves. This suggests high grade stenosis of the proximal left anterior descending artery.
