Noncardiac


A 25 year old previously healthy man who smokes presents with acute onset dyspnea and pleuritic chest pain. This was unprovoked and occurred at rest. EKG is shown above.

Challenge: What is this noncardiac diagnosis?

Noncardiac

This is a primary spontaneous pneumothorax of the left lung. There is low lateral precordial voltage on the EKG and a vertical or rightward axis shift.
